Dr. Hazelwood

Dr. Hazelwood is a board certified family physician who graduated from Kansas City College of Medicine and Biosciences in 2003. After completing her internship, she was called to active duty by the United States Navy where she served as a U.S. Naval Flight Surgeon until 2008. She served around the world with forward deployed, overseas and stateside units. After her military service, she returned to Tulsa where she completed residency at the University of Oklahoma, Tulsa.

Dr. Hazelwood is originally from Oklahoma City and graduated from the University of Oklahoma, Norman. She now resides in the Tulsa area. She has always been passionate about health, wellness and helping her patients feel and be their best! In her spare time, she enjoys traveling, sports, music and spending time with her family, friends and four leggers.
